QUALIFICATIONS:

- The willingness to work as a team member with a variety of individuals and take directions from supervisors.
- Must have, or be able to obtain, and maintain a valid Kansas driver’s license.
- Must have and maintain a dependable means of transportation.
CERTIFICATE / LICENE REQUIREMENTS:
Have and maintain a license to practice as a Physical Therapy Assistant.

JOB GOAL:
To enhance and maximize the responsibilities and scope of services of the supervising Physical Therapist.
PERFORMANCE RESPONSIBILITES:
- To provide direct treatment as outlined by the supervising Physical Therapist, adapting program as directed.
- To keep accurate and timely notes of treatment, session, and student response. Record pertinent information from teachers, parents, etc.
- To work closely with supervising physical therapist. Consult on a routine basis, provide access to student PT notes, etc.
- To attend appropriate workshops and in-services to further education.
- Submit required Medicaid/third party billing documentation as directed.
- Maintain proper license/certificate.
- Maintain a log of activities as directed to meet local, state, and/or federal guidelines.
- Perform any other duties as directed.
PHYSICAL ABILITIES:
- Ability to lift as required by job.
- Exhibit manual dexterity to dial a telephone, enter date into a computer, see and read a computer screen, hear and understand speech at normal classroom or recess levels.
- Ability to move between schools, school buildings and multiple rooms.
TERMS OF EMPLOYMENT:
Per “Memorandum of Employment Agreement.”
EVALUATION:
Performance of this job will be evaluated by the Executive Director and/or Designee as per board policy.
